Registration Instructions

  1. Please read these instructions.
  2. Click on the link below to fill out both required forms.
  3. Print the forms and attach to your quilt.

Instructions — Please fill out both forms for each item entered in the show and attach to your quilted item(s). Retain Section D for proof of entry and bring when picking up your quilt, or give to the person picking up your quilt.

Quilt Entry Form (link below)—
Section A will go to WVU for quilt insurance. If you have official paperwork from having your quilt appraised, please put that amount as the Appraised Value. Otherwise, please note the Replacement Value (cost of materials only). Replacement Value does not include the time spent making the quilt.
Section B will stay with the Quilt Show Committee.
Section C will remain attached to quilt at all times.
Section D will be required to pick up your quilt after the show.

Viewer’s Choice ribbons will be given in two categories:
1 - Bed Quilts (including crib quilts)
2 - Other quilted items

Display Tag (link below) will be pinned to your quilt during the show. Please type or write legibly so viewers can read.

Quilts must be picked up at the Quilt Show,
promptly at 5:00 pm on Sunday.
